Please complete your request below and we will endeavour to get your prescription to your chosen chemist within 24 hours. Please note this facility is for repeats of long-term medications, not for one-off requests.
This facility is for REPEAT Prescriptions ONLY and cannot be used for once off medication requests. If you require a prescription for a once off condition you must contact the clinic regarding same.
Your repeat prescription will be sent directly to your chosen pharmacy for collection.
To avoid an interruption in your supply of medication, please submit your request at least 7 days before your next prescription is due.
